Application fees are non-refundable per IDAPA 24.36.01.202.02. In addition due to internal controls funds are non-transferable. Please ensure that you are submitting the correct application and that a NEW application needs to be submitted. Renewing does not necessitate the submission of an application. If you are unsure, please call our office for clarification. 208-334-2356.

ATTENTION MILITARY SERVICE MEMBERS, VETERANS, AND SPOUSES
Pursuant to Idaho Code 67-9305 applications for members of the military, veterans and their spouses will be expedited. Please identify yourself by emailing our office at info@bop.idaho.gov once your application has been submitted. The email subject line should read 67-9305. EXPEDITED APPLICATION.

Pharmacist Student Interns

Pharmacist Student Intern Registrations must be renewed annually by July 15th. The renewal fee will be waived if renewed on time for the duration of student’s enrollment in the school or college of Pharmacy.

Certificate of College Enrollment – Certificate of College Enrollment Form
Fingerprint Card – For applicants 18 and older
